I hereby give Washington University in St. Louis (“University”) and its agents the right and my permission to record my appearance and participation on digital recordings, videotape, audiotape, film, still photographs, or any other medium (“Media”).  I understand that the University intends to use such Media on its website and, in particular, the webpage of Equity, Diversity, and Inclusion. 

I hereby grant to the University, in perpetuity, the right to use, publish, reproduce, modify, distribute, and publicly exhibit the Media, without restriction or limitation, for promotional and marketing purposes.  I understand that the Media may be used in a variety of materials such as brochures, publications, University webpages, and presentations, and other marketing or promotional literature and give my permission for the University to use the Media in any such materials and for any lawful purpose related to its educational mission.

I understand and consent to the University’s use of my name, likeness, voice, and other identifying information in connection with its use of the Media.  I agree that all Media taken, and any reproductions thereof, (including applicable copyright) shall remain the property of the University.  I waive any right to inspect or approve the Media or copy of the Media that may be used, as well as the use to which it may be applied.  I understand neither I nor my heirs and assigns shall be entitled to any compensation for the University’s use of the Media or my name, image, or likeness, in whatever manner they are used. 

I hereby release and forever discharge Washington University in St. Louis, its trustees, agents, officers, and employees, from any and all claims and demands arising out of, or in connection with, the use or distribution of the Media including, but not limited to, any claims for invasion of privacy, appropriation of likeness, violation of copyright, or defamation.

FERPA RELEASE FOR STUDENTS:  I understand that the Media may be protected as educational records under the Family Educational Rights and Privacy Act of 1974 (“FERPA”).  As a student, I understand that identifying Media generally cannot be used without the prior written consent of any student appearing therein.  I hereby authorize the University to publish and release the Media in its publications, on its websites, or any other marketing and promotional materials.  I further understand and acknowledge that the Media may be viewed by faculty, staff, students, and visitors to the University, as well as the general public.  I understand that the purpose of this disclosure and use is to advance the educational mission of the University.  

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name.  This release is binding upon me and my heirs, assigns, and personal representatives.  This consent shall remain in effect until revoked, in writing, by me.
